IVF is the newest and most successful form of IVF assisted reproductive technology. It is used to help fertility in male and female infertility, that is, infertility. During IVF, eggs developed using medication are collected from the ovaries when they mature and are fertilized with sperm taken from the spouse in a laboratory environment. Normally, fertilization takes place in the body, while in vitro fertilization takes place. Therefore, it is called "in vitro fertilization". Then, the embryos formed are placed in the uterus when they reach a certain size.
The duration of IVF treatment varies according to the protocol to be applied, but it takes approximately 15-20 days. Sometimes this process can be prolonged due to drug treatments covering 2-3 months according to different protocols.
Although couples want children within one year and do not use contraception, the condition of absence of pregnancy is called infertility. IVF treatment is used in infertile couples who cannot have children if other methods (vaccination, egg development, etc.) fail or are not suitable for the patient. In vitro fertilization is recommended as the primary treatment for women, especially 40 years and older. Apart from this, it is used in infertility of unknown cause, in those with damaged or blocked tube, in those whose tubes are tied, in those with ovulation impairment, in those with low ovarian reserve, in some female diseases such as endometriosis, in sperm number-movement or deformity in men, or in the absence of sperm in the semen. Sometimes, couples do not have problems getting pregnant, but because of a genetic disorder, they cannot have healthy children. In this case, genetic testing is applied to the embryos obtained by making in vitro fertilization. Embryos without genetic disease are selected and placed in the uterus and a healthy baby is tried to be obtained. In vitro fertilization may be an option for patients who need surgery, radiation or chemotherapy that may harm fertility due to cancer or other serious health problems. In appropriate patients, eggs are collected by stimulating the ovaries before cancer treatment is started. In married patients, they are fertilized with sperm taken from their spouses and after the cancer treatment is over, they are frozen and stored for transfer. In those who are not married, the egg is frozen. Likewise, in men, sperm can be frozen and stored. It can then be used when pregnancy is planned.
Before the IVF starts, some tests and scans are done. Although these vary according to the patient, in general, ovarian reserve tests and hormone tests are requested to determine the amount and quality of eggs in women. Uterus and ovaries are evaluated by ultrasound. Based on these results, the response of the ovaries to the IVF drug is estimated and treatment is planned accordingly. Male semen analysis is done and it is checked whether there is enough sperm for IVF. Apart from these, tests on whether there are some infectious diseases and tests depending on the special conditions of the patients may be requested.
In vitro fertilization treatment consists of placing the embryo formed in the uterus by combining the eggs taken from women and the sperm taken from men. IUI (intrauterine insemination) is a simple, low-risk method used to treat infertility. In this method, the sprinkler is washed and intensified, and the healthier and more active ones are selected. These are then released into the uterus, near the eggs, at the time of ovulation via a catheter. In some cases, cathter can also be applied to the cervix or vagina. However, the success of this type is lower than the IUI.
Today, the most commonly used method is microinjection method (ICSI). While microinjection is used only in couples with high sperm count or abnormality, ICSI comes to our mind directly when it comes to IVF.
